The following items are amendments to the final
version of the SEA-ORCHID Post-Intervention
Data Collection Manual (dated 17 December 2007).
Amendment 1 [posted 21 February 2008]
1. Q2 - MOTHER'S DATE OF BIRTH
What to do if the day/month/year
of the mother's date of birth is missing or
unknown.
Data collection form: write 'uk' in
the box for DD, MM and/or YY.
Online data entry:
1) if the day, month and year are missing: tick
unknown box
2) if only the day and month
are missing, enter: 30/06
3) if only the day is missing, enter: 15
2. Q34 – CORTICOSTEROIDS – GA of
most recent dose
What to do if the gestational age of the most
recent dose is unknown.
Data collection form: write ‘uk’
in the box
Online form: an ‘unknown’

The following items are amendments to the final version of the Manual for Data Collection Form and online data entry system (dated 11 February 2005 ).
Amendment 1 [posted 25 February 2005]
Q3 - mother's weight - what to do if the mother's weight is known but the gestational age is unknown.
Data collection form: write 'uk' in the 'gestational age' box.
Online data entry: write 'uk' in the 'gestational age' box. Note that if the mother's weight is also unknown, simply check the 'unknown' box.
Amendment 2 [posted 8 June 2005]
Q12 - the level of perineal trauma should be recorded irrespective of whether the trauma results from a tear or an episiotomy.
